Our Policies

• 1. HIPAA Notification

We understand the importance of privacy, even while vacationing abroad. Our practice has implemented the Health Information Portability and Accountability Act (HIPAA) to protect the privacy of our patients’ health information. Consent for Treatment and Patient Health Information forms will be required at each visit. The parent or legal guardian accompanying the child to the medical visit usually signs these forms. If a non-parent or non-legal guardian accompanies the child to the medical visit, we will require that the accompanying adult be listed on the Patient Profile as a person with permission to provide the Consent for Treatment and Patient Health Information forms. The non-parent or non-legal guardian will be required to show a photo I.D. that identifies him/her as an individual listed on the Patient Profile. If identification of the accompanying adult does not correspond with the information in the Patient Profile, the parent or legal guardian will be contacted.

• 2. Office Visit Hours

Regular Office Hours M-F: 8:30am to 2pm & 4pm to 7:00pm

• 3. Insurance Reimbursement

Monetary payment will be due at the time that medical services are rendered. Most medical insurance carriers provide short-term (“vacation”) emergency medical coverage while traveling abroad. Patients with short-term emergency medical coverage, or other applicable medical insurance coverage, should be prepared to provide an insurance card at the medical visit. Upon payment, we will provide you with an insurance “super-bill”, displaying a copy of your insurance card, which you may submit to your insurance carrier for proper monetary reimbursement as permitted per your policy benefits.
